Required Documents

Tenants and landlords should apply as soon as possible so their application can begin to be processed. During the course of your application, you’ll need to gather some of the below documents. If you don’t have these on hand or need assistance, program staff will be available to help.

Applicants will not be asked about their citizenship, nor will they be required to show proof of citizenship.

Tenant Required Documents

What documents will a Tenant need to provide to complete the application?

Tenants will need copies, pictures, or screenshots of the following types of documents/items (full checklist here):

Proof of Identity

All household members listed on the rental agreement must provide one government issued ID

  • Driver’s license

  • Government issued birth certificate or identification card

  • Employment identification card

  • Marriage license/certificate or certified divorce decree

  • U.S. Military photo ID

  • Tribal photo ID

  • Passport

 

Proof of Current Household Income

This might include:

  • IRS tax forms

  • W-2 forms

  • Recent paycheck stubs

  • Wage statements

  • Unemployment compensation statements

  • Interest statements

  • Receipts of benefit income (such as TANF, Social Security, SSI, and SSDI)

  • And more

Proof of Loss of Income

Or proof of significant costs incurred due to COVID-19, which may include the following items:

  • Declaration of COVID-Related Financial Hardship

  • A letter of termination from your job

  • Most recent pay stub with employer’s information

  • Proof that you have applied for unemployment benefits

  • Proof that your unemployment benefits have expired, including unemployment benefits provided through the CARES Act

  • For those self-employed: tax records, income statements, or other evidence showing loss of income

  • Other items will be considered

 

Proof of Residency

This may include:

  • A lease agreement

  • A government issued library card

  • Utility statement from providers, etc.

  • For those renting under a month-to-month agreement a certification that they still reside in the rental unit

Proof of the Amount of Rent Owed

This might include:

  • A current lease signed by the applicant and landlord or sub-lessor 

  • Bank statements that show the pattern and amount of rent, etc. 


Landlord Required Documents

What documents will a Landlord need to provide to complete the application?

Landlords will need copies, pictures, or screenshots of the following types of documents/items (full checklist here):

Proof of Ownership

Can include

  • Property deed

  • Mortgage note

  • Tax record

  • Copy of Property Insurance Statement

  • If applicant is a Property Management Company or Legal Representative, applicant must also attach documentation showing authorization to act on behalf of the property owner like an Authorization Agreement Contract/Memorandum of Understanding (MOU)/Memorandum of Agreement (MOA)

Proof of Identity

Can include:

  • Driver’s license

  • Government-issued photo identification

  • U.S. Military photo ID

  • Tribal photo ID

  • Passport

Financial Documentation

  • IRS W-9 Form

  • ACH Direct Deposit form

Proof of Tenant’s Residency

This may include: 

  • A lease agreement

  • A government issued library card 

  • Utility statement from providers, etc.
